Forms are a flexible feature that allows you to collect data in a more convenient way, they can also be used to display information to a customer which requires some sort of acknowledgement, Example: a patient consent form may include a video or information that a patient has to sign before a procedure can be complete.
Its important to lock legally binding forms so they cannot be edited, each business is responsible for using forms correctly in accordance with their own industrial governing body.
How to create a new Form?
Click Forms on the left side
Click Create Form on the upper right side of the page
Select a form type
Select a communication template (This acts as a cover letter and is what the patient will see before they are taken to the form via the included link)
Add a Form Title and a brief description of the form
Add Form Fields
Static fields β used for collecting data that is stored under the patient's record within a form, these fields don't update a patient's information with the following exclusion, checkboxes can be linked-to alerts, this is optional and will only update a patients information if checkboxes once they been linked.
Dynamic fields β Update the corosponding fields in the patient records
Enable contains medical data to update the last medical data tag under a patients profile, this is generally used to give an indicator to staff allowing them to quickly see the last time a medical history was taken without navigating to the dedicated forms area.
