Forms are used to collect information in a way that is convenient and easy for patients. They may also be used to share important details that require patient acknowledgement and a signature before a procedure can proceed.

For patients who require additional support from staff, forms can be completed internally using an iPad at the time of their appointment, ensuring they feel comfortable and supported throughout the process.

1. Navigate to the Patient File > Forms tab, then click New to create a new form.
2. Select the relevant form, choose Complete Form Internally instead of sending it to patient, and click Save.
3. Once the form has been created, click Send Link
Then, switch to the Internal Link tab > Create Link
4. Follow the step-by-step instructions to complete the form internally. Staff can use a tablet or iPad to scan the QR code shown, enter the code in Step 2, then hand it over to the patient to fill in the form.

5. Once the form has been successfully submitted, staff can review the patient details and accept any changes by clicking Review.
